Procedures for changing corporate information of graveyards, ossuary and crematoriums

Overview

When we change representative, location of corporation which has obtained management permission of graveyard, ossuary, crematorium, report of change is necessary.

Owners of graveyards, ossuary and crematorium

Content that can be changed in this procedure

  • Changes in representatives, locations, and names of corporations that operate graveyards, ossuary, and crematorium
  • Changes in manager of graveyards, ossuary and crematorium

Procedure Flow

1. Prior confirmation

Please contact Medical Care Bureau Health Sanitation Division for the information currently listed in the cemetery, ossuary, and crematorium before making a notification of change.
One notification will be made for one graveyard, ossuary, and crematorium. If you have more than one permission, please let us know.

The Medical Care Bureau Health Sanitation Division also gives you an offer.
If the corporate representative is an administrator, please fill in the changes.

Certificate of Registration of Corporation

Those that indicate that the change has been made (certificate of history, etc.)
※Certificate and issued within six months from the date of issue

This guide summarizes the outline of the change procedure and an example of entry for the change notification.

Notification method

Please submit change registration form and attached documents to ward office Health Sanitation Division window of ward where graveyard, ossuary, crematorium to change. When submitting by mail, be sure to specify the Phone number, which can be contacted during the day, and send it by a method that remains a record, such as a special record mail. If the application cannot be accepted due to attached documents or incomplete information, we may contact you to resend the documents.
※If you wish to return a copy of registration form, please enclose a stamp on the return envelope.

Attention

What I explained here is a general procedure.
In addition, please be sure to consult with Medical Care Bureau Health Sanitation Division in advance as procedures and documents to be submitted may differ depending on the content of the change.
We do not rewrite and issue permission letter by notification of change of application items. If you would like to keep a record of receiving the change notification, please make a copy separately from the submission and present it. I'll return it by pressing the reception stamp.
