Overview
When you change the representative, location, etc. of a corporation that has obtained a management license for a graveyard, ossuary, or crematorium, notification of the change is required.

Flow of Procedure
1. Preliminary confirmation
Please contact Medical Care Bureau Health Sanitation Division about the contents currently listed in the Cemetery, Ossuary and Crematorium Ledger before creating a notification of change.
For one graveyard, ossuary, and crematorium, there is one report. If you have more than one permission, please let us know.

This guide summarizes the outline of the change procedure and examples of entry in the notification of change.
Notification method
Please submit the change registration form and attached documents to the ward office Health Sanitation Division window of the ward where the graveyard, ossuary and crematorium is to be changed. When submitting by mail, be sure to specify the Phone number, which can be contacted during the day, and send it by a method that leaves a record, such as by mail. If the application cannot be accepted due to attached documents or incomplete information, we may contact you to resubmit the documents.
※If you wish to return a copy of registration form with the reception stamp, please enclose a stamp attached to the return envelope.

In addition, the procedure and documents to be submitted may differ depending on the content of the change, so be sure to consult with Medical Care Bureau Health Sanitation Division in advance.
We do not rewrite the permission letter by notification of change such as application items. If you want to keep a record of the receipt of the change notification, please make a copy separately from the submission form and present it. I'll return it by pressing the reception stamp.
