This is an online application procedure for obtaining certificate of family register (family register Certificate of All Matters) and certificate of partial family register (family register Personal Information Certificate). It is available to those who have a My Number card with permanent domicile in the city. We accept 24 hours a day. It will take about 10 days to deliver.

People who can use it
A person who has a permanent domicile in Yokohama and has a My Number Card with a digital certificate for signature.
Notes
- Online application can only be used by the person who has a My Number card.
- Person who does not have my number card Window requestOr Request by mailPlease use it.

What I need
1.My Number Card
- My number card isDigital certificate for signatureIs in an effective state.The PIN of the digital certificate for signature(Only if you know 6 to 16 digits alphanumeric characters (* not 4 digits)).
- If you have not updated your My Number Card information when moving in the past, you will not be able to apply online.
2.Smartphone or PC and card reader
Smartphone

- "Yokohama e application" (read my number card) application is necessary for use.
PC + card reader
- It is necessary to install an application for personal computers (electronic signature extension AP).

3.Credit card or smartphone payment
- Supported brands are VISA, Mastercard, American Express, JCB, and Diners Club.
- Smartphone payments are supported by PayPay and LINEPay.
Number of days until receipt
We will deliver it to Address, where resident certificate is located, in about a week after payment of the commission.
Notes
- If you are in a hurry, please consider using express delivery.
- It will take some time because the issuance procedure is not performed on weekends, holidays, and the year-end and New Year holidays.
- If you have any questions about the application details and need to be confirmed, we will contact you by phone or email. In that case, please note that it will take more days to create the certificate than usual.
Fees
450 yen per copy + postage
Notes
- For applications subject to fee reduction, please use the mail request etc.
- We will not issue receipts or other documents notifying you of receipts. Please check the credit card company or smartphone payment statement.
- If payment cannot be confirmed within one week from the date of sending the fee payment request email, the application will be canceled.
How to apply
- We perform user registration of Yokohama-shi electronic application, report system beforehand.
- Enter required items from the application page.
- Read your My Number card with a dedicated application, make an electronic signature, and complete the application.
- After confirming the application details at the Yokohama City Mail Request Office, you will receive an e-mail for a fee payment request. From My Page, proceed to the payment screen of the fee and make the payment.
- After confirming payment information at Yokohama-shi mail request office work center, we mail certificate.

- If the information on the digital certificate does not match the user registration information, you cannot use the online application. Please correct the user registration information to the correct information.
