The “Notice of Change of Residential Display” representing the new Address is a certificate required for Address to change procedures.
・Mailed to the head of the household in mid-September 2019
・6 cards are issued for each person aged 16 and over living in the district
If the number is insufficient or if you are under 16 years old and need a certificate, please contact the 22nd floor on the 2nd floor of the Midori Ward Office on the 2nd floor after Monday, October 21, 2019. (Free).
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office center.

Certificate of change in permanent domicile

The "permanent domicile Correction Notice" is a certificate required for procedures to change permanent domicile, such as rewriting of a driver's license in permanent domicile.
・Mailed to the leader of family register around late October 2019 (after October 21).
・Issued one sheet to family register, whose domicile is located in the district
If the number is insufficient, please request a "Certificate of Change of Land Name (permanent domicile Change Certificate)" (free of charge) at the 24th window on the 2nd floor of the Midori Ward Office after Monday, October 21, 2019. Please give me.
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office center.

About procedure of my number card (personal number card), notice card

Person who has my number card (personal number card)

The notation of Address will be changed due to the indication of residence in the second district of Nakayamacho, Midori-ku, so the procedure for changing the Address on the card face is required.
Please proceed with the procedure while looking at the bookmark (PDF: 3,360KB) (page 8).

Those who are applying for a My Number Card (Personal Number Card) from now on

With house indication enforcement of Nakayama-cho second district of Midori-ku, notation of Address is changed, but please apply for my number card using grant application attached to notice card that everybody had I can.

The notification card was abolished on May 25, 2020, so there is no need to change the Address notation.
