◆Certificates required for changes to Address and permanent domicile
<Certificate of Address Change>
New Address's "Notice" will be a certificate for changing Address.
- Mailed to the head of household around mid-August 2015
- 6 cards are issued for each person aged 16 and over living in the district
- If the number is insufficient or if you are under 16 years old and need a certificate, at the 202 window on the 2nd floor of the Izumi Ward Office on the 2nd floor after Monday, September 7, 2015, Please request free).
- Application for change of residence indication (PDF: 49KB)
<Certificate of permanent domicile Change>
The "permanent domicile Correction Notice" is a certificate for procedures for changing permanent domicile, such as rewriting of the driver's license permanent domicile.
- Mailed to the leader of family register around mid-September 2015
- Issued one sheet to family register, whose domicile is located in the district
- If the number is insufficient, please request a "Certificate of Change of Land Name (permanent domicile Change Certificate)" (free of charge) at the 202 window on the 2nd floor of the Izumi Ward Office after Monday, September 7, 2015. .
