The “Notice of Change of Residential Display” representing the new Address will be the certificate for Address's change procedure.
Mailed to the head of household around mid-September 2018
6 cards are issued for each person aged 16 and over living in the district
If the number is insufficient or if you are under 16 years old and need a certificate, please contact the 22nd floor on the 2nd floor of the Midori Ward Office on the 2nd floor after Monday, October 22, 2018. (Free).
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office center.

Certificate of change in permanent domicile

The "permanent domicile Correction Notice" is a certificate for procedures for changing permanent domicile, such as rewriting of the driver's license permanent domicile.
Mailed to the leader of family register around late October 2018 (after October 22).
Issued one sheet to family register, whose domicile is located in the district
If the number is insufficient, please request a "Certificate of Change of Land Name (permanent domicile Change Certificate)" (free) at the 24th window on the 2nd floor of the Midori Ward Office after Monday, October 22, 2018 .
When you are requested by mail, please send application and reply envelope (reply address entry, stamp attached) to Yokohama-shi mail request office center.

For those who wish to apply for a My Number Card after the date of the house indication

With the indication of residence in the first district of Nakayama-cho, Midori-ku, the notation of your Address will change on October 22. Therefore, we cannot use grant application of my number card attached to notification card which has already been sent.
