If you have an insured card for the Kanagawa Medical Association for the Elderly Medical Care, you will be issued an insured card issued at the time of simultaneous renewal of the insured card scheduled to be implemented in August 2024 (expiration date: July 31). Even after the health insurance card has been abolished, please bring it without discarding it until the expiration date.

※If you are 75 years old from August 2024 to November 2024, you will receive an insured person's card from the Kanagawa Medical Association for the Elderly Medical Care the Association before you reach the age of 75.

For those who are 75 years old after December 2, 2024 and do not have a My Number Card, or those who have not applied for the use of My Number Card's insurance card, "Qualification Confirmation Form (Same as the current insurance card) Size) ". By presenting the “Qualification Confirmation Form” to medical institutions, you can still receive insurance medical treatment as before.

If you are applying for a My Number Card insurance card, you will be issued a "Notice of Qualification Information (A4 size)" that allows you to easily check your own qualifications.

A． Even if it can be used as a health insurance card, information with high privacy such as consultation history and drug information, does not enter the IC chip of the card. If you drop or lose it, we accept temporary suspension of the card by toll-free 24 hours a day, 365 days a year.
