About medical expenses notice

Once a year, the Yokohama National Health Insurance sends a “medical expenses Notice” to the head of the household, indicating the total amount of medical expenses for all households who have visited medical institutions.
The medical expenses Notification is sent to help you deepen your awareness of the medical expenses burden structure of the National Health Insurance and your health (it will not be sent unless there is a patient in your household).

About medical expenses notice and declaration of medical expenses deduction in 2024
The medical expenses notification from January to December 2024 will be sent out on February 12, 2025. It will be delivered to you two days after the shipment to one week later.
This medical expenses notice is available for filing a medical expenses deduction. However, some consultations may not be described in the notice, or the names of medical institutions may not be listed. In that case, it will be necessary to supplement the medical expenses notice and prepare a separate statement of “medical expenses Deduction” and keep the receipt after the declaration.
You may also need to correct the amount of medical expenses paid.
In accordance with the enforcement of a law that partially amends the employee health insurance Law, etc. on October 1, 2020, it was prohibited to request the insured person identification number for purposes other than the execution of the employee health insurance business or related affairs.
Therefore, when sending a medical expenses notice to the tax office for medical expenses deduction, please mask your insured card number by yourself.

About browsing of medical expenses notification information on minor portal
You can now browse medical expenses information after the medical treatment in September 2021 on the minor portal. (You can browse from two months after the consultation month.)
Specifically, please see minor portal (outside site).
The medical expenses information provided on the minor portal does not include medical expenses such as judo reduction (osteopath, etc.).
In addition, depending on the timing of the creation of medical expenses information, etc., there may be differences from the medical expenses information described in the “medical expenses Notice” prepared by the city.
About automatic input of medical expenses deduction in tax return through minor portal
From the 2021 income tax return, it is now possible to automatically enter medical expenses notification information through the minor portal in the medical expenses deduction procedure.
※As for medical expenses related to medical treatment from January to August 2021, "medical expenses Notice" and receipts prepared by Motoichi will be required as before.

Due to the insurance medical treatment system, the consultation information of medical institutions can be confirmed in Yokohama City at least two months after the medical treatment month. For this reason, since data processing, printing, and enclosing work on computers will be started in February, when information for medical treatment in December can be confirmed, notices cannot be sent in time before the final tax return starts. Thank you for your understanding.
If you do not receive it after the sending period, please contact the ward office of your ward (bottom page).
If you declare the medical treatment described in the medical expenses notice with the original medical expenses notice attached, you do not need to keep the receipt in principle.
However, please note that in the following cases, it may be necessary to supplement or keep the receipt based on the receipt etc. when making the declaration. . For more information, please check the NTA website or contact the tax office in charge of your area.
(1) In the case of consultation at a medical institution outside the prefecture, some medical institution names may not be listed.
(2) Some consultations may not be listed due to delays in claiming National Health Insurance from medical institutions.
(3) Medical treatments not covered by insurance are not included in the medical expenses Notification.
(4) The amount paid to medical expenses in the medical expenses notice is calculated based on the medical fee statement (receipt).
It may differ from the actual amount paid at the counter.
(5) The amount of the refund will not be reflected if the refund is received through grants from high medical costs or medical expenses.

You can reissue the medical treatment for 2023.

Sending a medical expenses notice to the head of the household corresponds to the provision of personal information to a third party, but since it is difficult to confirm the intentions of all members in advance, if you do not notify us that you do not agree, we will judge that you have obtained your consent in accordance with national guidelines. If you do not agree to send the medical expenses Notification to the head of household, please contact the Insurance and Pension Division National Health Insurance Section of each ward office.
