Notice of medical expenses (medical expenses)
Delivery time of medical expenses announcement (medical expenses notification) for each year
- Medical treatment from January to November is mid-February of the following year
- December medical treatment is in mid-March of the following year
Declaration of medical expenses Deduction
The medical expenses Notification can be used to declare the medical expenses deduction. The medical expenses Notification can be used as attached documents in the "medical expenses Deduction Statement" (from 2018). However, if there are any items that are not listed in the medical expenses notice or the names of medical institutions other than Kanagawa and Tokyo are not printed, it is necessary to prepare a “medical expenses Deduction Statement” based on a separate receipt and attach the statement to the declaration form. (In this case, the receipt of medical expenses must be kept for 5 years from the deadline for final tax return etc.)
In addition, the amount of self-pay equivalent described in the notification may differ from the amount of the receipt, but it may be calculated based on the amount stated in the notification.

Declaration of self-medication taxation
The self-medication tax system (exception of the medical expenses deduction) allows individuals who take “certain measures” to maintain and improve health and prevent illness from January 1, 2017 to receive an income deduction for the purchase cost of OTC drugs.

Issuance of "Documents that clarify that certain efforts have been made"
In order to declare the application of the self-medication tax system, "documents that clarify that certain efforts have been made" are required.
Those who have visited Health Checkups in Yokohama can receive a certificate.
Fill in the name of the claimant, fill in the seal, and fill in the bold frame, and submit it to issue a certificate. If you would like to issue a certificate, please contact your ward office Insurance and Pension Division National Health Insurance Section.
