Payment of medical expenses (when refund is available)
In the following cases, you will have to pay the entire amount once. Please apply to the ward office National Health Insurance Section with necessary documents such as insurance card and application form later.
If the National Health Insurance reviews and finds it unavoidable that insurance could not be used, 70% of the insurance coverage (80% before elementary school, 80% or 70% for those aged 70 and over) Will be refunded.
Please note that it will take about 2-3 months to be paid for the examination.
※If you want to transfer money to an account other than the head of household, you need private seal (the name of the head of household using vermilion).
※Please note that two years after payment to medical institutions, etc., the statute of limitations will expire and you will not be able to apply.
Case 1 When insurance cannot be used due to emergency or other unavoidable circumstances, such as sudden illness (when the insurance card cannot be brought)
(Note) Limited to medical treatment in Japan. For overseas medical treatment, please refer to Case 5 below, "When you receive treatment at a medical institution due to illness or injury overseas."
What is required for application
- Receipt
- Medical fee statement that shows the name of the illness and the details of the medical treatment (requires a request to issue it to a medical institution)
- Medical (except Dentistry, oral surgery)

Case 2 When a treatment device such as a corset is made
What is required for application
- (1) Doctor's instructions (certificate)
- (2) Receipt and Statement
※Except for treatment glasses for elastic clothing and children with amblyopia, (1) is the date when the insurer confirms the wearing of the treatment equipment, and (2) the name of the prosthetics and orthotics who was involved in the manufacture of the equipment is required. - When a shoe orthosis is made, a photograph of the orthosis, etc. (It can be confirmed that the patient actually wears.)

Case 3 When receiving treatment by a judo reduction teacher (if you present your insurance card by "reception commission payment", you may only need to pay a partial contribution)

Case 4 With the consent of the doctor, when receiving a beam, moxibustion, or massager's treatment (If you present your insurance card by "reception commission payment", you may only need to pay a partial contribution) )
What is required for application
- Receipt
- A statement that shows the details of the treatment
- Doctor's Consent Form
- Health insurance card
- Copy of bank passbook or account number etc.

Case 5 When treated at a medical institution due to illness or injury overseas
(Note 1) Travel for therapeutic purposes is not eligible.
(Note 2) Medical practices that are not covered by insurance in Japan are not eligible.
What is required for application
- (1) Receipt
- (2) Receipt statement and statement that shows the name of illness and medical treatment
- (1)Japanese translation in (2) and (2)
- The original passport (passport) of the person who received the treatment (* 1)
※1 If you cannot confirm the fact of your travel on your passport, you must request the Ministry of Justice to disclose your immigration record and submit a certificate of immigration. - Consent Form (*2)
※2 The contents of the application may be confirmed with a local medical institution, etc., so please fill out the consent form of the person who received the medical treatment.

※ When applying, an application is required in addition to the above "Required for Application". You can find the necessary documents in your ward office, National Health Insurance Section. In addition, we can download from National Health Insurance medical expenses, special medical expenses supply application (PDF: 136KB) [example] (PDF: 153KB).
※Out-of-pocket expenses (partial contributions) may be covered by high medical costs.
※Doctor's consent is required when undergoing treatment by a judo reduction teacher due to fracture or dislocation.
※"Receiving commission payment" is the handling of paying a partial contribution (20%, 30%) at the time of treatment and entrusting the receipt of the remaining amount to the practitioner.
※For insurance medical treatment, after applying, we will examine the contents of the medical treatment and compare the amount replaced when receiving insurance medical treatment in Japan with the actual cost locally. The co-payment amount is calculated based on the cheaper one, and the excess amount is paid as overseas medical expenses. Please note that it will take 2-3 months to be paid for the examination.
※Overseas, medical expenses is very low or expensive compared to medical treatment in Japan. Therefore, please note that the cost paid locally and the amount paid are not necessarily proportional.
