Medical institution Check ups for Infants and Toddlers

Use the consultation slip (medical institution Check ups for Infants and Toddlers consultation card) bound in the spelling of the medical examination ticket handed when the Mother and Child Health Handbook was issued.
You can receive three Health Checkups free of charge at medical institutions.

Health Checkups and childcare consultation costs will be borne by public funds.
However, please note that the cost of treating the disease is not included.

You can consult three times by 12 months (less than 13 months) after birth.

Please use it within the validity period indicated on the consultation slip spelling of the medical examination ticket handed when the Mother and Child Health Handbook was issued.

※As we do not tell about medical examination individually, please be sure to consult a medical institution in Yokohama within the validity period.
※We do not reissue consultation slips due to loss, etc. Please be careful not to destroy the spelling of the medical examination ticket after childbirth.

Medical institutions in the city can be consulted only at Check ups for Infants and Toddlers conducting medical institutions.
In addition, since the implementation date varies depending on the medical institution, please check in advance before using.
※Medical examinations are not available at medical institutions outside Yokohama City.
※Reimbursement payment (refund) for expenses when consulted outside the city is not possible.
