Medical institutions Check ups for Infants and Toddlers

Use the consultation card (medical institution Check ups for Infants and Toddlers consultation card) bound in the spelling of the medical examination ticket handed when the Mother and Child Health Handbook is issued.
You can get three Health Checkups free of charge at a medical institution.

Expenses for Health Checkups and childcare consultation will be borne by public funds.
However, please note that the cost of treatment for illness is not included.

You can have a medical examination three times by 12 months after birth (less than 13 months).

Please use it within the validity period indicated on the medical examination ticket spelled out at the time of issuing the Maternal and Child Health Handbook.

※As we do not tell about medical examination individually, please consult medical institution in Yokohama-shi within validity period by all means.
※We do not reissue consultation slips due to loss or other reasons. Please be careful not to discard the spelling of the medical examination ticket after giving birth.

Medical institutions in the city Only available at Check ups for Infants and Toddlers medical institutions.
In addition, the implementation date varies depending on the medical institution, so please check in advance before using.
※Medical examinations are not available at medical institutions outside Yokohama.
※Reimbursement (refund) of expense when we consult outside the city is not possible.
