- Elementary school connection addition implementation report (Excel: 48KB)

Please submit to the ward Children and Families Support Division where the facilities and offices are located by the 15th of the month in which the change was made.

※If you use the Yokohama City Child / Child Care Billing Statement Creation Software, you do not need to enter it because it is output from the system.
※Please submit by e-mail if there is a change in the registration information such as a new establishment, a new system transition garden, or a transfer account.
