Last Updated October 14, 2021
The guidelines for entering and leaving special elderly nursing homes are scoring the situation of applicants who need nursing care and higher scores for residents of Yokohama City.
There are five main items to measure the need for admission. Each of these items will be scored, and admission will be determined from the person with the highest total score.
・Degree of nursing care required for those who enter
・Status of applicants
・Situation of the family who is the primary caregiver
・Residents of Yokohama City
・Other special notes
The "degree of nursing care required" is 5 at 40 points, and 1 is 5 points.
"Status of applicants for admission" is 15 points for those living alone.
"The situation of the family who is the main caregiver" is that there is no caregiver.
"Yokohama City Residents" will be added to those who have a long-term care insurance card issued by Yokohama City.
"Other special notes" are other items that add items that are considered to be highly necessary for admission.
