Welfare and nursing care
A

Under the system to reduce the burden on users by social welfare corporations, households exempt from municipal tax are presented with a reduction confirmation certificate when using the target The Long-term Care Insurance service, so that 10% of the user's burden is 25%, and food and living expenses are reduced by 25%.
In addition, living expenses for those who receive social security will be reduced by 100% when using private rooms.

Subject requirements

(1) Municipal tax-exempt households who fall under all of the following a to o
Oh, income standard
Single-person households must be less than 1.5 million yen per year (500,000 yen added for each additional household member).
B. Asset Standards
Single-person households must be less than 3.5 million yen (the sum of 1 million yen for each additional household member).
Do not own real estate other than residential land (less than 200m2) and houses.
D) Not being supported by relatives who have the ability to bear the burden (family and relatives have not received dependent deductions for the person).
You must not be behind with Long-term Care Insurance Premium.

(3) Short-term entrance life care [short stay] (including services pertaining to the Yokohama City Life Support Short Stay Business Implementation Guidelines)
(4) Visiting care (including services pertaining to the Yokohama City Home Life Support Home Help Project Implementation Guidelines)
(5) Care prevention short-term entrance life care
(6) Preventive care Dementia responsive outpatient care
(7) Prevention of long-term care Small-scale multifunctional home care
(8) Night-time home-visit care
(9) Dementia-responsive outpatient care
(10) Small-scale multifunctional home care
(11) Community-based nursing homes for the elderly
(12) Regular patrol / ad-hoc home-visit nursing care
(13) Nursing small-scale multifunctional home care
(14) Community-based day care
(15) Among the first visit business, the business equivalent to the old care preventive visit nursing care (the self-pay ratio is limited to the same as insurance benefits.)
(16) Out of the first tsusho business, the business equivalent to the old care prevention tsusho care (the self-pay ratio is limited to the same as insurance benefits.)

Procedure

(1) Please contact the Health and Social Welfare Bureau Elderly Facilities Division (Phone: 671-4901, FAX: 641-6408) and request application documents.
(2) Please submit the application documents to the Health and Social Welfare Bureau Senior Facilities Division by mail.
(3) If you are eligible, you will be sent a “Certificate of Reduction of User Burden by Social Welfare Corporation” by ordinary mail (no forwarding required).
(4) When "user burden reduction confirmation certificate by social welfare corporation" arrives, be sure to present it to the company. If you do not present it, you will not be able to receive relief.
