About entrance application of Yokohama-shi special elderly nursing home

In Yokohama-shi, we set up "special elderly nursing home entrance application reception center" to plan convenience of those who applied while ensuring fairness, transparency about procedure of entrance application, and accept application collectively doing.
The order of entrance is not the order of application, but the "Yokohama City Special Nursing Home Entrance and Exit Guidelines" (PDF: 282KB) is set so that those with high necessity and urgency can enter, and based on it, We score degree of care required, household situation, and guide entrance from person with higher score.

Please mail the following submission documents to the "Special Nursing Home for Nursing Home Application Reception Center".
Before filling out the entrance application form, please read the entrance application guide (PDF: 1,202KB) carefully beforehand, and fill in the person who knows the physical and mental condition of the person. Priorities may be changed if the facts are found to be different.

※We distribute application in ward office Elderly and Disabled Support Division, community care plaza, special elderly nursing home.
※Due to the review of the entrance and exit guidelines in April 2024, new information will be required for entrance applications, so the contents of the application form used so far will be changed. Applications after March 21, 2024 are valid only for the new form currently being released. Please note that if you apply in the old format, you will need to resubmit.

• About change, please mail application A, B and copy of care insurance card to application reception center in the same way as new application.
  If the contents of the application form, such as "degree of nursing care required" and "the situation of the applicant / family", change, it may be related to the change in rankings.
• If only the certification period for long-term care is renewed and there is no change in other situations, please mail only a copy of your long-term care insurance card to the application reception center.
• If you would like to withdraw your application, please contact the application reception center.
