Please obtain the application form on the The Long-term Care Insurance website of Health and Social Welfare Bureau. After filling in, please mail it to the "Admission Application Reception Center" with a copy of your long-term care insurance card.

Depending on the application status of the special elderly nursing home that has already been opened, it will be waiting for vacancy. Information on the application form submitted to the entrance application reception center will be provided to the desired facility, and the priority will be determined by the entrance examination committee of each facility in descending order of "need for nursing care". "Necessity of nursing care" is determined by scoring the contents of the application form (developing standards at each facility in accordance with the Yokohama City Entrance and Exit Guidelines). If the situation changes after application (change of degree of nursing care required, relocation, death, etc.), please contact the entrance application reception center.

The Long-term Care Insurance will pay the user's burden (10%, 20% or 30% of nursing care costs), food, living and daily living expenses to the facility. No deposit is required.
