Eligibility requirements in this proposal shall be those who meet all of the following items:

1 On the date of submission of the proposal participation intention application form, it must be registered in "Relationships and Consignment (Reiwamoto, List of Qualified Persons for 2nd Year)" and registered business events as follows. Or, at the time of submission of the bid participation application form, qualification screening is currently underway, and registration has been completed by the date of determining the trustee. Register “Various survey projects” in the first place as a business event, and register or review as B “Consultation (excluding construction consulting, etc.)” as a detailed business event.
2 Have a track record of supporting industry-academia collaboration projects in the health and medical fields (*) and consulting for practical application of research results.
Person who has not received suspension measures by rule of "measures summary such as Yokohama-shi nomination suspension (April 1, 2004)" from presentation deadline of 3 proposal participation intention proposal book to specific day of trustee .
4 Being a person who does not fall under the provisions of Article 167-4 of the Local Autonomy Law Enforcement Ordinance (1947 Cabinet Order No. 16).

（※） Health and medical fields refer to drug discovery, medical device development, diagnostic technology development, preventive medicine, regenerative medicine, nursing care and welfare, and health services.

Applicants who have been notified that they have not been eligible for the proposal may request a written explanation of why the proposal was not accepted. In addition, the document must be submitted to the participation intention application form submission destination by 5:00 pm, five days after the city hall closing date, from the day following the day on which the city sent the notice. Motoichi will respond in writing to those who have requested explanations within 5 days excluding the city hall closing date from the day following the day on which the above document was received.
