No. 6 certification (bankruptcy of financial institutions)

Accreditation requirements

Please refer to the website of the Small and Medium Enterprise Agency (external site) for failed financial institutions designated by the government.

You must meet all of the following requirements at the same time.

  1. There must be a business establishment in Yokohama City.
  2. Balance of loans to bankrupt financial institutions designated by the government. (Bill discount is not eligible.)

  1. Documents (copies are acceptable) that can confirm the actual business conditions in Yokohama City and the borrowing status at the time of the latest declaration

    • Corporate
      • Certificate of all history items ※The latest version within three months of issuance
      • The latest set of income tax returns
    • Sole proprietorship
      • The latest blue tax return financial statements or a set of income tax final income tax return (those that indicate that the tax office has accepted the receipt, e-mail details (in the case of electronic filing), etc.)

※The tax return contains all of the cover, balance sheet, income statement, statement of income, breakdown of borrowings and interest payable, or statement of accounts.


  1. Documents to confirm the borrowing balance and borrowing period from failed financial institutions (all of the following documents are required: Copy is acceptable)
    • “Balance Certificate” issued after the date of bankruptcy and within one month prior to the date of application for certification.
    • Repayment (repayment) schedule
    • Contracts concerning borrowing of money, etc.

※All borrowings eligible for the application amount are required.

Application method

It is recommended that you apply for a financial institution. Please ask the financial institution that provides consultation on loans.


Window application (reservation required)

  • Please make a reservation in advance from the window reservation application form, and tell us your "name (or company name)" at the reception.
    ※We do not accept reservations on the day. In addition, if you arrive at the counter more than 15 minutes after the reservation time, we will not be able to accept it.
  • In the case of a proxy application by a financial institution.List of agency applications (Excel: 70KB)(2 copies) Please prepare it and bring it with you.
