Please apply for deemed designation (visit nursing, visit rehabilitation, home care management guidance) from the link below.

※If you apply for designation by the designated date: Sending designated notices, etc. around the 1st of the following month
※When applying for designation after the designated date: Send designated notices, etc. around the first day of the month following the application or two months after the application (depending on the application date).
※If you decline the designation: There are no particular items to be sent.
