After applying for designation using the electronic application system, please arrange the required documents on the notification page and submit it by mail immediately.

If you have already been designated by an insurance medical institution, but have not received the deemed designation at that time, or if you have answered "reject" to the intention confirmation from Yokohama City, a new application for deemed designation Will be done by mail.
The same applies when a nursing care facility for the elderly is designated as a home rehabilitation.
