※If you apply for designation by the designated date: Sending designated notices, etc. around the 1st of the following month
※When applying for designation after the designated date: Send designated notices, etc. around the first day of the month following the application or two months after the application (depending on the application date).
※If you decline the designation: There are no particular items to be sent.

About establishment which calculates addition

The following documents are required to calculate the addition at the same time as the new designation.
After submitting a designated application using the electronic application system, please arrange required documents on the addition notice page and submit it by mail immediately.

●Status list of systems related to nursing care benefit calculation

●When designating home rehabilitation at a nursing care facility for the elderly

If you have already been designated by an insurance medical institution, but have not received the deemed designation at that time, or if you once answered "decline" to the intention confirmation from Yokohama City, a new application for deemed designation will be made Will be mailed.
The same applies to nursing care facilities for the elderly receiving designation for home rehabilitation.
