About care plan that needs report

Home care support establishments need to report a care plan to Yokohama City in the following cases.

From October 2018, from the viewpoint of user independence support, prevention of seriousness, effective use of local resources, etc., the insurer will provide care plans for care plans where the number of uses of life support-centered services in home-visit nursing care exceeds the standard number of times Notification is required.

2. Copy of home service plan "Table 1" to "Table 7"

• Please issue the home service plan "Table 1" to the user and submit the one with a signature.
• Home care support progress "Table 5" may submit only a page that describes the reason for positioning home-visit care.
• Unify the paper size to A4 size.

3. Copy of home-visit care plan

• Please submit thing that designated home care support establishment (care manager) received from visit care establishment.

• The results of the inspection will be announced by facsimile or e-mail at a later date.

For each home care support establishment unit, the usage ratio of the category payment limit standard amount is 70% or more, and more than 60% of the use service extracts home care support establishments that are "visit nursing care services", of which 2021 In care plan which we made or changed after October 1, thing which Motoichi appoints is targeted for report.

About care plan that report is necessary, we appoint in Motoichi according to the preceding paragraph and notify applicable home care support establishment individually.
This notice is scheduled for February 2022 and beyond.
