About care plan that needs notification

Home care support establishment needs to report care plan to Yokohama-shi in the following cases.

From October 2018, from the viewpoint of user independence support, prevention of seriousness, effective use of local resources, etc., insurers will be informed of care plans in which the number of uses of life support centered services in home-visit care exceeds the standard number. Notification is required.

After October 1, 2018, based on the home service plan issued with the consent of the user for the following reasons 1 to 4, it was positioned as home-visit nursing care more than the standard number of nursing care required in Table 1 Please report about thing by the last day of the next month.
Example) If created in October, a notification must be made by the end of November.

2. Copy of home service plan "Table 1" to "Table 7"

• Please issue home service plan "Table 1" to user and submit thing with signature.
• Home care support progress "Table 5" may be submitted only on the page that describes the reason for positioning home care centered on life support. .
• Please unify the size of the paper to A4 size.

3. Copy of visiting care plan

• Please submit thing which designated home care support establishment (care manager) received offer from home care establishment.

From October 2021, with the aim of contributing to the creation of a care plan that can lead to the provision of home-visit care that matches the intentions and conditions of the user, the usage ratio of the category payment limit is high, and Of care plans with a high percentage of home-visit care to the service, notification to the insurer is required for those designated by Motoichi.

For each home care support establishment unit, home care support establishments where the usage ratio of the category payment limit standard amount is 70% or more, and more than 60% of the use services are "visit nursing care services", of which 2021 In care plan which we made or changed after October 1, 2021, thing which Motoichi appoints is targeted for report.

About care plan that report is necessary, we appoint in Motoichi according to the preceding paragraph and notify applicable home care support establishment individually.
This notice is scheduled for February 2022 or later.
