   Home care support offices in Yokohama, etc. (Eligible plan is one establishment)
   
3. Details of implementation
   (１) Written check of care plan
   (２) Interview with Chief Care Manager of Yokohama City care manager Council
   　①Online (Use Zoom)
   　②Face-to-face (Yokohama City Hall Conference Room)

   (３) A report summarizing the review (sent after the interview)

   1st term: Monday, April 1, 1960 to Tuesday, April 30, 2006　　
   2nd term Wednesday, May 1, 1960 to Friday, May 31, 2006　　
   3rd term: Saturday, June 1, 1960 to Sunday, June 30, 6
   4th term: Sunday, September 1, 1960 to Monday, September 30, 2006　

※In the case of a large number of applications, the deadline may be before the deadline.
※It is also possible to apply for establishments implemented in FY2019 4 and 5 years, but depending on the application status, we may not be able to meet your request, such as giving priority to unimplemented establishments.
