- If the claimant is a company, the application must have a company sealed seal. You may also be required to present a copy of the resident certificate.
- About copy of resident certificate with resident certificate code
If you would like to know your resident certificate code, please take a copy of the resident certificate with the resident certificate code. Requests require public documents such as a driver's license and employee health insurance that can confirm your name. In addition, the person who can claim is limited to the person or the same household employee.

resident certificate certifies certain matters (Address, head of household, name, date of birth, gender, etc.) in the prescribed form.
※If you can prove your Address or your date of birth, and you don't want to provide extra personal information, such as the former Address, please use the resident certificate certificate instead of a copy of resident certificate.

- If you do not have a private seal registration card (Seal Registration card), you will not be able to issue a certificate. Please note that a certificate cannot be issued even if the registered private seal or your license is presented.
- Please write the Address listed on the resident certificate correctly on the application form. You may need the name and room number of the apartment or apartment. If you are applying for proxy/agent, please check in advance before visiting the Agency.

※1 This is necessary when you are going to register for relocation or name change when real estate registration or car registration has been made in Address before the indication of residence.
※2 At the time of the house indication, those who were not in Address cannot be proved. In that case, you can perform the transfer registration and name change procedure by obtaining a certificate of non-resident status or absence from the registered Address.
It proves that "the Address of the certificate does not have resident certificate" as of the date of certification.

In order to prevent third parties from impersonating themselves and obtaining certificates and to further protect personal information, we will verify the identity of applicant when applying for a certificate such as a copy of family register, an abstract, or a copy of the resident certificate.
※If you do not show your identity verification materials, you may not be able to issue a certificate, so please prepare it in advance.
