Application forms and guidance (for tax payment)

1 Not all application and notification forms are provided.
This page provides only the application form for issuing certificates, etc., which are often used for tax purposes. About style which we do not offer, please use thing which we distribute in presentation (ward office) of application.

2 Applications and notifications cannot be made via the Internet such as e-mail.
Please apply and submit using the submission method displayed in detail of application / notification procedure.

3 "Adobe Reader" is required.
Application and notification styles are provided in PDF (Portable Document Format). To view and print PDF files, Adobe Reader, which is distributed free of charge by Adobe Systems Incorporated, is required. If you do not have it, download and install Adobe Reader (external site) from the Adobe Systems Incorporated site before using it.

4 Use plain paper for printing and print in black.
Please use white plain paper of the style size (A4 size, etc.) displayed in the application / notification procedure details for printing the application / notification form. Also, print it out in black. Please note that if the size and printing surface are different, it may not be accepted. Please do not use thermal paper, backing paper, colored paper, etc.

5. Please note the following points when printing the application / notification form.
When printing the application / notification form, display the form on the screen and press the print button on Acrobat Reader. You cannot print the application / notification form with the print button on the browser.
