About municipal tax, prefectural tax, forest environmental tax (special collection) delivery form

When you pay municipal tax, prefectural tax, forest environmental tax (special collection) and municipal tax, prefectural tax (Retirement income) to Yokohama City, please use the delivery form specified by Motoichi.

1. Download the form (Excel) of the delivery form.
2. Enter the item of the yellow cell in the "Input Sheet".
3. After confirming the contents of the "input sheet", print the "print sheet".
4. The printed delivery form is a three-layered sheet. Separate each and bring it to the counter of a financial institution as a set of three sheets.

• For Phone number, enter the contact information that can be reached from the area code.
• If there is an error in the designated number, you will not be able to confirm the payment. The designated numbers in Yokohama City are 2 and 5-digit numbers 7 digits. We list designated number in tax amount letter of advice sent from Yokohama-shi.
• If you do not know the designated number, please contact the Finance Bureau Tax Administration Division.
• If the corporate name and location are different from the tax amount letter of advice sent from Yokohama City, please submit the "Location Name Change Notification" together.
• The deadline for delivery is 10th of the month following the “Delivery Target Date”. (If the 10th falls on a Saturday, a holiday, or a holiday, the day after a holiday or a public holiday)
• If you want to pay monthly salary deductions, enter the amount in the salary column.
• When paying municipal tax, prefectural tax, and forest environmental tax on Retirement income, please enter it in the Retirement income column. In addition, please input and print the “Delivery Declaration Form” sheet and send it separately to the Finance Bureau Tax Payment Management Section.
• There is an error or omission in the input sheet of the "print sheet" and the "input sheet has an error or omission in the input sheet. If it is displayed, return to the "Input Sheet" and correct.
