Request a certificate of acceptance at the counter

This is the procedure for obtaining the receipt certificate at the counter. A certificate of acceptance is a certificate that proves that the family register has received a marriage, birth, divorce, etc. You can get it at Family Registry Division, the ward office where you submitted it.

A person who can claim

You can request applicant of the notice.
applicant is the person who signed seal in the applicant column of the family register notice.
If another person makes a request at the request of applicant, a letter of attorney from applicant is required.

※You can also request mail.

What I need

1. Invoices for family register Certification, etc.

2. Identification documents of the person coming to the counter

  • Driver's license
  • Passport (passport)
  • My number card (personal number card) or Basic Resident Register card with photo
  • Resident Card or Special Permanent Resident Certificate

Family Registry Division of the ward office that submitted the notification

Monday-Friday, 8:45-17:00
From 9:00 a.m. to noon on the 2nd and 4th Saturday
※Excludes public holidays and New Year holidays
About ward office window opening agency of the second, fourth Saturday

Notes

We cannot request except ward office which made report.

Fees

350 yen per copy
※You can use cash or electronic money (transportation, WAON, Rakuten Edy, nanaco) to pay the fee.

When creating with high-quality paper (B4 size)

1,400 yen per copy
Only marriage, divorce, adoption, adoption separation or acknowledgement notification. It will take a few days to issue.
