The Children's Day Care Support Program provides nursing care to elementary school student with disabilities and elementary school student who are concerned about their development.

elementary school student Development Support | We provide individual nursing programs based on individual support plans, such as training to learn basic movements in daily life and adapt to group life. Applicable to pre-school children with disabilities and children under the age of 18 who do not have a school register. |

Consultation support for children with disabilities
Consultation support specialists provide necessary support to children with disabilities who use daycare services for children with disabilities, such as adjusting the use of services, checking the usage status (visiting offices) and providing information on services, so that they can receive comprehensive consultation on the growth of elementary school student and life after entering society. In addition, we will create a plan for using support for children with disabilities and regularly monitor the usage status of services.
In order to use the tsusho support project for children with disabilities, you will need a certificate issued by Children and Families Support Division, your ward. Please note that it takes about two weeks to one month to issue a recipient ID in order to conduct an interview survey and confirm usage requirements and determine the amount of payment after applying to each ward's Children and Families Support Division.

[1 Use Consultation]
Please consult with your ward Children and Families Support Division or the consultation support office for children with disabilities about the use of daycare support for children with disabilities.
※With child with a disability consultation support establishment, we can talk about how to use service and are establishment to make use plan.
[2 Visit to Support Centers for Children with Disabilities (hereinafter referred to as Offices)]
Please check with the office directly for availability. Also, if you can confirm the availability, please visit in advance.
[3 Submission of Application Forms, etc.]
We submit "application such as payment of child with a disability place payment costs" and "use plan for child with a disability support or Yokohama-shi child support plan" to each ward Children and Families Support Division.
(Depending on the service you use, you may need to submit other documents.)
[4 Interview Survey]
The person in charge of the ward office will talk directly about your growth history, your child's condition, whether you meet the usage requirements, and how many times you want to use it a month. We will examine whether the payment is appropriate.
[5 Issuance of Recipient ID]
As a result of the examination, if the payment is deemed appropriate, the recipient ID will be mailed to your home.
[6. Contracts and Use Starts with Business Operators]
Present the recipient ID to the office and sign a contract. After that, we will receive an explanation of the contents of the individual support plan and start using it.
Points when visiting business sites
When visiting the office, please check the details of the nursing care, daily schedule, staff status, other elementary school student usage fees, usage time, pick-up and drop-offs, etc.
About user burden
The person who uses the service will bear 10% of the usage cost, but the maximum monthly amount is set according to the income, and no further burden will occur regardless of the amount of service used in one month. ※Depending on the establishment you use, you may be charged actual expenses such as snacks.

About Yokohama-shi version day service (elderly day care) guidelines after school
The Ministry of Health, Labour and Welfare has established guidelines for day service (elderly day care) after school, etc., and based on national guidelines, Yokohama City has established the Yokohama City version of the after-school day service (elderly day care) Guidelines, which quotes the actual situation and examples of Yokohama City and adds as easy-to-understand interpretations as possible.
