The daycare support program for children with disabilities provides nursing care to elementary school studenｔ with disabilities and elementary school studenｔ, who are concerned about their development.

Consultation support project for children with disabilities
Consultation support specialists provide necessary assistance to children with disabilities who use daycare centers for children with disabilities, such as adjustingelementary school studenｔ's use of services, checking usage status (visiting offices), and providing information on services. In addition, we prepare a plan for the use of support for children with disabilities and monitor the usage status of services and other services regularly.

In order to use the tsusho support project for children with disabilities, you will need a certificate of recipient of a child with a disability issued by your ward Children and Families Support Division. After applying to each ward Children and Families Support Division, please note that it takes about two weeks to one month to issue a recipient ID to conduct an interview survey to confirm usage requirements and determine the amount of payment.

We submit "application" and "child with a disability support use plan or Yokohama-shi child support plan" to each ward Children and Families Support Division.

　The person in charge of the ward office will ask you directly about whether you meet the usage requirements and how many times you want to use it a month. We will examine whether the payment is appropriate.

[5 Grant of recipient ID]
　As a result of the examination, if the payment is determined to be appropriate, we will mail the recipient ID to your home.

[6 Contracts and Start of Use with Business Operators]
　Present the recipient ID to the office and make a contract. After that, we will receive an explanation of the contents of the individual support plan and start using it.

When visiting the office, please check the details of nursing care, daily schedule, staff members, other usage elementary school studenｔ usage fees, usage time, and whether or not there is a transfer.

The person who uses the service will bear 10% of the usage fee, but the maximum monthly fee is set according to the income, and there is no further burden regardless of the amount of service used in one month. ※Depending on the establishment you use, actual expenses such as snacks may be incurred.

※From October 1, 2019, the user's burden of services such as elementary school studenｔ Development Support for preschool children with disabilities has been free of charge.

The Ministry of Health, Labour and Welfare has established guidelines for the after-school day service (elderly day care), but Yokohama City has established the Yokohama City version of the day service (elderly day care) Guidelines for After-school etc. based on national guidelines, citing actual conditions and actual examples of Yokohama City and interpreted as easy as possible.

After the start of use, the company may ask you to create a "day service (elderly day care) Evaluation Table after School" (questionnaire), so please cooperate.

From April 2018, the national system was revised to conduct appropriate evaluations of the day service (elderly day care) after school.
