Consultation support for persons with disabilities

　For all people who wish to use disability welfare services and community consultation support, we will provide support based on usage plans such as services, for the realization of the desired life. The contact point for use is Health and Welfare Center (Elderly and Disabled Support Division) of each ward office.

A self-plan is a self-plan and adjusts the use of services without using plan consultation support, if the person with a disability wishes.
