Established in April 2021 with the aim of applying the remuneration unit price according to the type of business establishment and the staffing system so that appropriate personnel can be assigned and the system can be continued when receiving medical care from nurses etc. at a business establishment.

In any of the following cases, it is necessary to determine the medical care category.
①Use When the main target of elementary school studenｔ is establishments for severely handicapped children, and when using establishments that calculate the "additional nursing staff" in order to allocate more nurses, etc.
②When using a business establishment that is not listed in 1 above and that can provide medical care by assigning nurses, etc. on its own.

①In case of: The prescribed unit will be added to the basic remuneration per use according to the situation of each business establishment (400 to 800 units).
②In case of: Depending on the content of medical care and watching, there are three categories of basic remuneration per use (604 units to 2,885 units).
In order to receive this medical care category decision, it is necessary for parents to ask family doctor (doctors who regularly examine their children) to create a "medical care score table" (* Please note that the document fee set by each medical institution involved in this preparation will be borne by the guardian.) 。

・The "Medical Care Score Table" needs to be confirmed once every 12 months.
・Therefore, in principle, when renewing the beneficiary ID, if it is necessary to determine the medical care category, it is necessary to submit a “Medical Care Score Table”.
・However, if the first day of the renewal date of the new recipient ID is within 12 months from the date of judging the new judgment score submitted last time, the new judgment score may be used until the end of that period. Is also acceptable.
・Please check with your establishment to see if you need a medical care category.
・If the judgment result of the “Medical Care Score Table” does not change, give a copy of the “Medical Care Score Table” kept at hand to the medical institution and enter the “Update Judgment column for Update” You can also have your doctor fill out.
