How do I apply for Medical Certificate for infant?

Yokohama City subsidizes co-payment (partial co-payment) for insurance medical treatment when a child who is enrolled in employee health insurance visits a medical institution due to illness or injury.
The Medical Certificate will be issued to those aged 0 to 3rd grade.
To apply for a Medical Certificate grant, please bring your child's health insurance card and apply at the Insurance and Pension Division Payment Section (26th window on the 2nd floor).
Applications for Medical Certificate are also accepted by mail.
You receive application at ward office the second floor 26th window (benefit charge window) or download the following application.
(* Please print the application form in A4 size.)
Please send the application form and a copy of your child's health insurance card in a sealed letter.
