This survey is based on a questionnaire survey of care managers belonging to home care support offices in the city, and the status of discharge adjustment and
Understand the actual situation of the “hospitalization / discharge support map” and the “home care support map for nursing care professionals”
The purpose is to do so.
We are pleased to announce that the results of the survey have been finalized.

| Investigation method | We send questionnaire to 867 home care support establishments in the city. Answered by anonymous method and collected at each business site. |

| Number of collected items | 1,855 responses (1,855 valid responses) |
