This survey was conducted through a questionnaire survey targeting care managers belonging to home care support offices in the city, and the status of discharge adjustment and
Understand the actual situation of the recognition of "hospitalization / discharge support map" and "home care support map for nursing care workers".
The purpose is to do this.
We are pleased to inform you that the results of the survey have been finalized.

| Survey method | We sent questionnaire to 863 home care support establishments in the city. Answers by anonymous method, collected at each business site. |

| Results of collection | Response offices 562 / 863 (collection rate 65.1%), responses 1,982 (1,982 valid responses) |
