Recruitment of subsidy for Yokohama-shi pediatric home-visit nursing training expenses subsidy

When a visiting nurse working at a home-visit nursing station attends training on medical care and nursing for children or newborns
I will support the tuition fee.

Home-visit nursing stations that provide home-visit nursing
※This subsidy is provided to provide new home-visit nursing at home-visit nursing stations that have not provided pediatric home-visit nursing.
　If you would like to apply for a grant, please contact us before attending the training course.

Within three months from the day after the payment of the tuition fee for the subsidized training

Fees for medical and nursing training for children or newborns
※We compare sum of half of expense and 40,000 yen and assist low sum.
※When fraction less than 1,000 yen occurs in calculation of subsidy, fraction less than 1,000 yen is rounded down.
※Only the training fee is eligible for the grant. (Expenses such as exchange meetings, lunch, and transportation expenses are not covered.)

Please submit the following documents by mail or email to the City of Yokohama, Medical Care Bureau Regional Medical Section Home Health Care Cooperation Section.
□ Grant application and results report (word: 25KB) ← Please download style from this and apply.
(The seal of the representative can be omitted.) If seal is omitted, you can submit it by e-mail.)
□ Documents that provide details such as the contents of the training to be subsidized, date of implementation, tuition fees, etc. (copy of course guidance and guidelines)
□ Documents certifying the payment of the tuition fee for the training to be subsidized (copy of receipts, etc.)
□ Documents certifying that you have completed the training to be subsidized (copy of completion, etc.)
※If you do not have a certificate of completion, ask the implementing organization to issue a certificate of completion or an alternative certificate (such as a certificate of participation).

□ Documents (pamphlets, copies of websites, etc.) that confirm that child home-visit nursing is being carried out
