Recruitment of 2024 Yokohama-shi pediatric home-visit nursing training costs subsidy

When a visiting nurse working at a home-visit nursing station attends training on medical care and nursing for children or newborns.
We will support the tuition fee.

Home-visit nursing stations that provide pediatric home-visit nursing
※This subsidy is provided to provide new pediatric home-visit nursing at home-visit nursing stations that have not previously provided pediatric home-visit nursing.
　If you would like to apply for a grant, please contact us before attending the training.

Within three months from the day following the day on which the tuition fee for the subsidized training was paid.

Tuition fee for training on medical care and nursing exclusively for children or newborns
※We compare half of expense with 40,000 yen and assist lower sum.
※If a fraction of less than 1,000 yen occurs in calculating the subsidy, fractions less than 1,000 yen will be rounded down.
※Only the training fee is eligible for the grant. (Expenses such as exchange meetings, lunch, transportation expenses, consumption tax, etc. are not eligible.)

Please submit the following documents to the following application / reference by e-mail or mail.
□ Grant application and results report (word: 25KB) ← Please download style from this and apply.
(It is possible to omit the representative's seal seal. If seal is omitted, it is also possible to submit by e-mail.)
□ Documents that show details such as the contents of the training to be subsidized, the date of implementation, and tuition fees (copy of attendance guidance and outlines, etc.)
□ Documents certifying the payment of the tuition fee for the training to be subsidized (copy of receipts, etc.)
□ Documents certifying that you have completed the training to be subsidized (copy of certificate of completion, etc.)
※If you do not have a certificate of completion, ask the implementing organization to issue a certificate of completion or a substitute certificate (such as a certificate of participation).
　(The name of the training, the name of the participants, and the date of completion are known.)
□ Documents (copy of pamphlets, websites, etc.) that can confirm that pediatric home visits are being carried out
