Yokohama City Home Healthcare Collaboration Base

In cooperation with the Yokohama Medical Association, Yokohama City provides support to doctors who are responsible for home medical care and medical support to care managers who are responsible for home care.
We maintain and operate “home medical cooperation bases” to be implemented in all wards, and enhance and strengthen home medical care and nursing care cooperation.
