Yokohama City Home Healthcare Collaboration Base

In collaboration with the Yokohama Medical Association, Yokohama City provides support to physicians in charge of home medical care and medical support for care managers in charge of home care.
We maintain and operate the “home medical care cooperation base” to be implemented in all wards, and enhance and strengthen home medical care and nursing care cooperation.
