In this hospital, we accept applications for clinical survey personal forms by document reception. Currently, due to the timing of renewal procedures for the specified medical expenses (designated incurable disease). In addition, we do not accept applications by mail.

If you come to the hospital only by applying for or receiving documents, please come to the hospital from 8:30 am to 9:30 am or from 1 pm to 5 pm, we will accept at relatively low waiting time I can do it.

※Reservations are not required when visiting the hospital only by applying for or receiving documents. Please come directly to the reception desk on the first floor.
※Currently, it takes about three weeks from application of the documents to handing over.
