About introduction of patients

There is no designated style in the letter of introduction (medical information report).
You can also use the unified style of "patient introduction letter" prepared by the Yokohama City Medical Association.

When you introduce, we recommend that you make a reservation so that you do not have to wait on the day of the consultation.
Anyone can make a reservation, but you will need basic information such as the patient's name, date of birth, Address and Phone number.
