About reservation
In this hospital, in order to shorten the waiting time for patients, reservations are required in principle.
If you wish to have a medical treatment, please make an appointment in advance.
Anyone can make a reservation, but you need basic information such as the patient's name, date of birth, Address and Phone number.
In addition, medical treatment may not be possible at the appointment time due to outpatient consultation status, emergency patients' response, emergency surgery, and sudden changes in in hospitalized patients. Thank you for your understanding in advance.

※Brain disease (headache, dizziness, convulsions, etc.) and spinal cord disease (low back pain, etc.) can be consulted without an appointment, but a doctor cannot be specified.
※In addition, reservations will be given priority, so if you are not booked, you may have to wait.
Non-insurance combined medical expenses (selected medical treatment)
If you do not have a letter of introduction from another medical institution at first visit (Shoshin), you will be required to pay 7,700 yen (tax included) as non-insurance combined medical expenses (selected medical treatment) in addition to the partial co-payment for insurance medical treatment.
In addition, if there is no reservation or continuous outpatient clinic for 3 months and you go to the hospital again, in principle, it will be treated the same as first visit (Shoshin), and if you do not have a letter of introduction from another medical institution, you will be charged 7,700 yen (tax included) .

- My Number Card insurance card (or health insurance card)
- Medical Certificate (if you have one)
- Letter of introduction, image data (if you have one)
- Osuzuri Notebook (if you have one)
- Medical card (patient's card Shinryo-ken) (if you have one)
※If you use the high medical costs system with your My Number Card insurance card, you do not need a medical ticket such as a limit certificate.
Flow on the day of the consultation
Fill out the application form
Please fill out the application form on the entry table prepared before first visit (Shoshin) reception desk.
When filling out in advance at home, etc., print out the medical treatment application form (PDF: 116KB) and bring it on the day of the consultation.
※Please use the medical treatment application form (Excel: 21KB) when entering.
first visit (Shoshin) reception desk
Once you have completed the application form, please submit the application form, insurance card, various Medical Certificate (incurable disease Medical Certificate, Independence Support Medical Certificate, etc.) and a letter of introduction to the first visit (Shoshin) receptionist.
We will create a medical chart with patient's card Shinryo-ken based on information such as the medical application form, so please receive it.
Outpatient reception
Please submit patient's card Shinryo-ken and medical records to the outpatient reception and accept them.
Medical consultation fee's questionnaire will be given to you, so please fill out the form, submit it to the reception desk, and wait for the order of the examination in front of the examination room.
When your turn comes, enter the consultation room and see a doctor.
The outpatient reception desk is located on the first floor and the rehabilitation outpatient reception desk is located on the first basement floor.
※If you wish to fill out the questionnaire at your home or other location in advance, please print out the questionnaire for the Medical consultation fee you are going to see.

Accounting
When all consultations and examinations are completed, submit a basic outpatient slip to the accounting reception and receive a receipt number slip.
If the number of the receipt number slip is displayed on the accounting information display board, please pay with the automatic payment machine.
You can choose to pay by cash and credit card.
