Please contact the Regional Collaboration General Consultation Office by telephone. After that, you will receive a letter of introduction (medical information report) from the hospital that is hospitalized or treated by fax.
After the doctor in charge decides to apply for hospitalization, we will inform you of the results of the judgment and inform you of the date of hospitalization when the bed is ready. If it is difficult to judge based on the information received alone, we may ask you to have a family member or individual outpatient consultation.
Patients and families who are hospitalized at other hospitals should consult through the hospital where they are hospitalized.
