Consultation on hospitalization

Please consult a hospital counselor who is currently hospitalized or treated to the Regional Cooperation General Consultation Office by telephone. After that, you will receive a letter of introduction (medical information report) and ADL slip from the hospital that is hospitalized or treated by fax.

Dr. Rehabilitation confirms the patient's medical information and makes a decision to adapt to the hospital.

After the doctor in charge decides to apply for hospitalization, we will inform you of the date of hospitalization when the bed is ready.

※If it is difficult to judge based on the information you receive alone, we may ask you to have a family member or individual outpatient consultation.
※For tours and other consultations and requests, please contact the Regional Cooperation General Consultation Office.
※In response to hospitalization consultations from hospitalized patients and family members, we will not be able to judge the adaptation of hospitalization, so please consult through the hospital you are hospitalized.
