Please submit the application form and reply envelope to the outpatient reception, or mail it to our hospital to apply.
In addition, if it is necessary to confirm the contents of the instructions, we will contact the patient and family, the person in charge of visiting nursing stations and home care support offices from our hospital, so please respond .

Please decide the office and ask family doctor to create a home-visit nursing instruction manual before applying.
Sheets are divided into "new" and "continuation". For patients who request the creation of a home-visit nursing instruction manual for the first time, please fill out the "new" sheet, and for patients after the second time, please fill out the "Continuation" sheet.
