Our hospital has obtained facility standards for home care logistics support hospitals since November 2022.

We provide medical treatment in cooperation with medical institutions that provide home medical care so that patients and their families who are being treated at home can spend time at home with peace of mind.
If medical treatment is urgently required due to a sudden change in the medical condition of a patient during home medical treatment, we will accept it 24 hours a day.
Target patients need to register patient information in advance.

・Those who live in homes, special elderly nursing homes, light-paid nursing homes, care houses, paid nursing homes, group homes, elderly housing, etc.
・Those who are receiving on-site medical treatment
・Home medical care comprehensive management fee at home, medical comprehensive management fee when entering a facility, home cancer medical comprehensive medical treatment fee, home medical treatment guidance management fee (excluding home self-injection guidance management fee) are calculated by the home medical institution in the month of hospitalization or the previous month Those who do.

Patient
It is necessary to register in advance through your GP as a "hospital that wishes to receive medical treatment / hospitalization in an emergency".

Doctors in charge of home
If the patient wishes to register, please prepare the “Home Care Logistics Support Hospital Registration registration form [Form 1]” and “Home Care Logistics Support Hospital Explanation and Registration Agreement [Form 2]”.
We will fax and mail it, so if you wish, please contact the Regional Cooperation General Consultation Office.
After receiving the form, please write required items, attach the “Medical Information Report Form (free format)”, and fax or mail it to the Regional Cooperation General Consultation Office.
