1. Applicants must be at least 20 years old as of the date of application
2. Being disabled at home (not entering a facility)
3. Have not been hospitalized for more than 3 months
4. Annual income must be below the standard
5. Must meet the following disability requirements:
   Being in a state that requires special care at all times in daily life due to severe severe mental or physical disability

[Initiation] ¥28,840 per month (as of April 2024)
※If approved, payment will be made from the month following the application date.
In February, May, August and November, the allowance up to the previous month will be paid.
[Necessary] Those whose personal number (My Number) can be confirmed
Identity verification documents (If a person other than the person submits a report, the person's identity verification document is also required)
Items that can confirm the income status, medical certificate for medical treatment (*), passbook of the individual's ordinary bank, etc.
※As a general rule, please submit a medical certificate in the prescribed format (within 3 months). For more information, please contact Health and Welfare Center in advance.
