I want to apply for a medical treatment certificate, what should I do? (For those who have undergone a positive diagnosis at a medical institution after September 26, 2022)

Regarding proof of medical treatment, please consider using alternative documents first.
In addition to the certificate of medical treatment, the Life Insurance Association shows the following examples of documents that can be used as alternative documents that can be confirmed to have a new coronavirus infection in addition to the medical treatment certificate. For more information, please see here (external site).
Please contact your insurance company for any documents that can be handled.

  • Those that show the results of PCR tests and antigen tests conducted at medical institutions.
  • Medical treatment statement (medical management fee includes "addition of hospitalization for patients with second-class infectious disease patients" (including temporary treatment for outpatient care and medical fees)
  • Prescription and instruction manual containing coronatherapy drug
  • Received results (SMS, LINE, etc.) of the Health Follow-up Center (Kanagawa Prefecture Positive Registration Desk) established by local governments
  • A copy of the email exchanged between the health center and the positive person
  • Guidance sentences issued by the public health center to positive persons (describe points to note about health observation and life support)
  • Test results (excluding commercially available test kits) of test centers (other than medical institutions) that conduct PCR tests and antigen tests

Those who have been diagnosed as having a new coronavirus infection at a medical institution after September 26, 2022 will be divided into "Notification target" and "Not applicable" in the notification of outbreaks from doctors. In addition, medical treatment certificate cannot be issued to those who are not eligible for notification of outbreak.
Those who are subject to notification of outbreaks from medical institutions to public health centers (= those who can issue medical treatment certificates) are as follows.

  1. 65 years old or older
  2. Pregnant
  3. Those who need to be hospitalized
  4. Those who are at risk of illness and whose physician determines that treatment of COVID-19 or need to administer COVID-19
