I want to apply for a medical treatment certificate, what should I do?

Yokohama-shi finished acceptance of issuance application of medical treatment certificate on Friday, May 31, 2024.
If necessary for insurance claims, please refer to the following link.
In addition to medical treatment certificates, the Life Insurance Association provides examples of documents that can be used as alternative documents to confirm that you have been infected with COVID-19. For more information, see "Click here" (outside site).
For documents that can be handled, please contact your insurance company.

  • Those that show the results of PCR tests and antigen tests conducted at medical institutions, etc.
  • Medical treatment statement (medical management fee includes "Inpatient medical treatment for Class 2 Infectious Disease Patients" (including temporary treatment for outpatient medical treatment and medical fees))
  • Prescription and medication instructions containing corona treatment drugs
  • Receiving results (SMS, LINE, etc.) of health follow-up centers established by local governments (Kanagawa positive person registration window)
  • A copy of the email exchanged between the public health center and the positive person
  • Guidance sentence given to positive people from the public health center (notes points to keep in mind for health observation and life support)
  • Test results (excluding commercially available test kits) at test centers that conduct PCR and antigen tests (other than medical institutions)
