How do I get a child Vaccinations outside of Yokohama?

If you receive regular vaccinations at a medical institution outside the city, you will need a “Vaccination Implementation Request Form” issued by The Mayor of Yokohama. (In principle, vaccination costs will be borne by yourself [paid])
About issuance of "vaccination conduct request", please apply in Health and Welfare Division Health Promotion Section of ward with resident registration.
