A single company (organization) that satisfies all of the following items, or a consortium whose members share the qualification requirements of 1 and all members meet the qualification requirements of 2 and 3. Have participation qualifications. In the case of a consortium, please submit a "Consortium Agreement and Power of Attorney" (Attachment Form 2) at the time of application. In addition, after identifying the contract candidate, please conclude a union contract that clearly defines the representative organization and the division of responsibility by the time the contract is concluded, and submit a copy of the union contract. In this proposal, a member of a consortium may be a member of two or more consortiums at the same time, and cannot be proposed as a non-consolidated company.

1. Regarding "Various Survey Planning A Market / Public Opinion Poll" and "Various Survey Planning B Consulting (Excluding Construction Consultants, etc.)" in the "Yokohama City General Competitive B Qualified Person List (Articles, Consignment, etc.)" Must be registered in 2nd or higher. Or, the application is currently underway at the time of submission of the application form, and the registration has been completed by the date of identifying the contract candidate.
2. From the deadline for submitting the participation intention application form to the specific date of the contract candidate, no suspension measures have been taken under the provisions of the "Yokohama City Nomination Suspension Measures Guidelines (April 1, 2004)".
3. Do not fall under the provisions of Article 167-4 of the Local Autonomy Law Enforcement Ordinance (Government Ordinance No. 16 of 1947).

Of those who have submitted the application form for participation, those who have been approved and those who have not been approved will be notified in writing of that effect and the reason. If you can confirm that you have a proposal qualification, we will send you a proposal-related document submission request form.

• Applicants who have been notified that they have not been eligible for the proposal may request a written explanation of why the proposal was not approved. In addition, please submit document to participation intention application form presentation by 17:00 five days after 17:00 except city hall closing day from the next day when Motoichi sent notice.
• When an explanation is requested pursuant to the preceding paragraph, Motoichi will respond in writing to the person who requested the explanation within 5 days excluding the closing date of the city hall from the day following the day on which the above document was received.

　Applicants who have been qualified for the proposal can view the materials listed in “7 Reference Materials” in the Business Explanatory Materials at the International Cooperation Division. When visiting, please contact us by phone the day before.
