Aiming at the 4th Yokohama City Community Welfare and Health Plan (plan period: Reiwamoto-5), citizens to realize "Let's make" Yokohama "where everyone can live in their own health with peace of mind" Through the plan, we create videos for the purpose of "being interested in the community" and "to have them think about what they can do for the community."

Those who intend to participate in this proposal shall meet the following requirements (1) to (3).
(1) As a result of the examination pursuant to the provisions of Article 7 of the Yokohama City Contract Regulations, one of the following business types that were listed on the list of qualified general competitive bidding participants for Reiwamoto and 2nd year and determined to correspond to the contract One must be within 3rd place.

(2) Person who has not received suspension measures by rule of measures summary such as suspension of Yokohama-shi general competition participation suspension and nomination suspension from presentation deadline of proposal participation intention application to specific day of trust candidate.
(3) Have been involved in video creation in the field of welfare and health in Yokohama City or other municipalities so far.

Questions and answers will be notified to all proposal-related document submission requesters. If you do not have any questions, you do not need to submit a questionnaire.

Of those who have submitted the application form for participation, those who have been approved and those who have not been approved will be notified in writing of that effect and the reason.
At the same time, we will send a proposal-related document submission request form by mail and e-mail.
Notification date: Shiped around Tuesday, April 21, 2020 (planned)

[Contents of Proposal]
(1) Proposals shall be prepared in accordance with the attached format (Form 5, Procedures-2-9).
(2) In principle, the size of the paper is A4 size (vertical and horizontal), horizontal writing, left binding, and double-sided printing. However, if it is necessary to consider visibility, etc. depending on the contents of the description, the page of A3 (if binding, insert it into the size of A4 size) may be included.
(3) For proposals, please describe the proposals for the following items in the prescribed format.

  • Business performance of corporations (last 5 years)
  • Business Implementation System
  • Business history, etc. of planned employees
  • Planning and composition (total story)
  • Effective dissemination to the target
  • Initiatives related to work-life balance and employment of persons with disabilities
  • Reference quotation
  • Application Form for Intention to Disclosure of Proposals (Guidelines 9)

(4) Please note the following items when preparing the proposal.

  • Please describe the way of thinking briefly using documents, figures, and tables.
  • Please display the number at the bottom of each page.
  • Multicolor printing is acceptable, but monochrome copy will be made for evaluation, so please consider the ease of viewing.
  • Characters should be 10 points or more in principle, except for notes, etc. If the number of pages is increased, the total number shall be within 12 pages except for Procedure-4.
  • Please consider and describe the use of technical terms so that you can evaluate objectively.
