Those who intend to participate in this proposal shall meet any of the following requirements (1) and (2) and satisfy (3).

(1)As a result of the examination pursuant to the provisions of Article 7 of the Yokohama City Contract Regulations, one of the following business types that were listed on the list of qualified general competitive bidding participants for Reiwamoto and 2nd year and determined to correspond to the contract Must be registered for more than one.

(2)Although it has not yet been listed in the list of qualified general competitive bidding qualifications for Reiwamoto and 2nd fiscal year, the same as (1) above, which was determined to respond to the contract when submitting the application form for participation The person who is currently applying for the business and is expected to complete the registration by the date on which the contract candidate is determined.

(3)Being a person who has not received suspension measures according to the provisions of the measures outline, such as the suspension of Yokohama City nomination suspension, from the deadline for submitting the proposal participation intention application form to the specific date of the contract candidate.

Notification of participation qualification confirmation result and request for submission

We confirm proposal qualification of participation intention application form and send participation qualification confirmation result notice (style 2) by mail and e-mail by around Wednesday, December 25, 2019 regardless of proposal qualification .
At the same time, a proposal-related document submission request form (Form 3) will be sent by mail and e-mail (only for qualified participants).

[Format and Contents of Proposal]

(1)Please make proposal based on attached proposal cover (style 4) (word: 22KB) and proposal style (the point-2-10) (word: 30KB).
(2)In principle, the size of the paper is A4 size (vertical and horizontal), horizontal writing, left binding, and double-sided printing. However, if it is necessary to consider visibility, etc. depending on the contents of the description, the page of A3 (fold it to the size of A4 size when binding) may be included.
(3)For proposals, please include proposals for the following items in the prescribed format.
(A) Business performance of corporations (last 5 years)
B. Operational Implementation System
C) Work history of planned employees
D. About assumed schedule
Construction of a mechanism for oprobono
F. Holding of training on how to proceed and management considerations for administrative staff and related support organization staff.

(4)Please pay attention to the following matters when preparing the proposal.
A. Please describe the way of thinking in a concise manner using documents and charts.
B. For each page, please display the page number at the bottom.
C Multicolor printing is acceptable, but monochrome copy will be made for evaluation, so please consider the ease of viewing.
D. Characters should be at least 10 points in principle, except for notes, etc., and should be created within a range that fits in the prescribed format.
(E) Consider the use of technical terms so that you can evaluate objectively.
