2 Specifications

(1) Medical institutions
 In principle, the designation of medical institutions located in Yokohama City is performed by the The Mayor of Yokohama (excluding medical institutions established by the government), but at the same time as reporting to the local welfare bureau regarding insurance medical institutions, etc. When we perform notification of designation application about designated medical institution, report of change, notice of abolition, notice of suspension, notice of resumption, and report of decline, we can omit separate report to Motoichi. (2) Practitioner
 The designated location changes depending on the practitioner's Address and the location of the practitioner. Please check the following notes before proceeding with the procedure.

※As the effect of the designation extends nationwide, if you receive designation in Yokohama City, you do not need to receive the designation of another prefectural governor (other mayor) again when you receive designation in Yokohama City.

About 3 report

(1) About unification of report to Health and Welfare Bureau
From  July 1, 2023, when submitting a report on insurance medical institutions, etc. to the local welfare bureau and submit various reports on social security designated medical institutions at the same time, omit the separate notification to Motoichi (excluding decline, etc.). This treatment applies to hospitals, clinics, Dentistry and dispensing pharmacies. About home-visit nursing establishment, practitioner, report to Motoichi is necessary as before.

(2) About application in Yokohama-shi electronic application, report system
 (1) About report which does not apply to, we accept various reports by Yokohama-shi electronic application, report system from January 11, 2024. User registration is required to use the service. In addition, if you perform the procedure using the electronic application / notification system, you do not need to submit a paper application form.
 In the use of the Yokohama City Electronic Application and Notification System, you can access it from the following link. When using the service, the medical institution must be registered as a "business" (the practitioner can also be used for "individual" registration). After that, enter "Welfare Protection Act" in the "Keyword Search" field and search. From the search results, select "designated medical institution" or "designated practitioner / midwife" from the search results.

(3) Regarding written application
 (1) If the report does not apply to the Yokohama City Electronic Application and Notification System cannot be performed, we will accept the written application as before.
