About designation of medical institution by the Public Assistance Act

 Hospitals, clinicss, pharmacies, home-visit nursing establishments (hereinafter referred to as medical institutions) and practitioners who wish to provide medical benefits to social security recipients must be designated under the Public Assistance Act. In addition, even after becoming a designated medical institution or designated practitioner, notification is also made when there is a change in the name location (name residence) of the designated medical institution and the designated practitioner, or when the business is abolished. 2. Specific contents

(1) Medical institutions
 In principle, The Mayor of Yokohama designates medical institutions located in Yokohama City (excluding medical institutions established by the government), but at the same time as notification to the local welfare bureau regarding insurance medical institutions, etc. When submitting a notification of designated application for designated medical institutions, application for designated renewal, notification of change, notification of abolition, notification of suspension, notification of resumption and notification of decline, a separate notification to Motoichi can be omitted. (2) Practitioner
 Depending on the location of the practitioner's Address and the practitioner's office, the designated location will vary. Please check the following notes before proceeding.

※Since the designation extends nationwide, there is no need to receive the designation of another prefectural governor (other mayor) again when receiving designation in Yokohama City or when treating protected people in other prefectures.

(1) Regarding the unification of notifications to the Health and Welfare Bureau
 From the application on July 1, 2023, when submitting a notification regarding insurance medical institutions, etc. to the local welfare bureau and submitting various notifications regarding social security designated medical institutions, separate notification to Motoichi can be omitted (excluding declines). This treatment applies to hospitals, clinics, Dentistry and dispensing pharmacies. About home-visit nursing establishment, practitioner, report to Motoichi is necessary as before.

(2) About application in Yokohama-shi electronic application, report system
