From July 1, 2023, some formats and notification methods related to designated medical institutions and practitioners have changed. For more information, please refer to the following link.

From January 11, 2024, we have been accepting applications using the Yokohama City Electronic Application and Notification System in principle. If you have completed this procedure, you do not need to submit a paper application form. For more information, please refer to the following link.

2. Where to submit documents and inquiries
Please submit it to the following address. In addition, the place to receive the designation differs depending on the location. For details, please see "2 Designated Contents" on the "About designation of medical institutions" page. Applications that do not use the electronic application / notification system are, in principle, accepted by mail. If there are any deficiencies in the documents, please contact us.
