About designation of care institution by the National Assistance Act, please see About designation of care institution for explanation of report contents.

From January 11, 2024, we have been accepting applications using the Yokohama City Electronic Application and Notification System in principle. If you complete this procedure, you do not need to submit an application form in documents.

・When we received designation by the Nursing Care Insurance Law on date before June 30, 2014
・When we received designation of Nursing Care Insurance Law after July, 2014, we declined deemed designation by the National Assistance Act.

Applications that do not use the electronic application / notification system are, in principle, accepted by mail. If there are any deficiencies in the documents, please contact us.

　A similar report by the Nursing Care Insurance Law must be made at the same time to perform various reports such as change, abolition, suspension, including designation of designated care establishment by the National Assistance Act.
　Please note that the contents of the report will not be reflected even if the notification is made under the Nursing Care Insurance Law without notification under the Nursing Care Insurance Law.
