Request for issuance of medical tickets and dispensing tickets
From October 1, 2024, we will start accepting requests for issuing medical tickets and dispensing tickets to the Health and Welfare Center Life Support Division in each ward using the Yokohama City Electronic Application and Notification System. In addition, when you cannot use Yokohama-shi electronic application, report system, please request issuance of conventional medical ticket, dispensing ticket. In addition, if you have been notified of the required period of a medical ticket in advance using a written opinion on necessity of medical care, an office contact form, etc., you do not need to apply at any time using the electronic application / notification system. If you do not receive it at the time of the lump-sum ticket issued by the City Life Support Division every month, please request.

How to request ticket issuance
Medical tickets and dispensing tickets can be requested using the Yokohama City Electronic Application and Notification System (outside site). For details, please refer to the following notice. In addition, if you have been notified of the required period of a medical ticket in advance by a statement of necessity of medical treatment, office contact form, etc., you do not need to apply at any time using the electronic application / notification system. Please request when you did not arrive at the time of lump-sum issuance that ward life support section performs every month.

If you cannot use the electronic application / notification system, please ask the ward Health and Welfare Center Life Support Division for various tickets in the same way as before. In addition, the following forms can be used when requesting the issuance of medical and dispensing tickets by fax or mail to the Health and Welfare Center Life Support Division and the Health and Social Welfare Bureau Life Support Division in 18 wards (support benefits for Japanese nationals remaining in China).
