Dementia-responsive cohabitation care

About registration form about Cooperating Medical Institutions

In accordance with the revision of the nursing care fee in fiscal 2024, designated dementia-response cohabitation care providers are required to confirm with Cooperating Medical Institutions at least once a year when the condition of the user suddenly changes, and to notify the designated right holder of the name of Cooperating Medical Institutions.
